First Visit

Your first visit to our office is usually about an hour in length. At this appointment we will gather information about you. Please be able to provide the following:

• Completed medical and dental health history
• Insurance information
• List of current medications you are taking
• Any recent x-rays from previous dental office (within a year old)

If necessary, we will take new xrays at this appointment and Dr. Abate will do a thorough examination including oral cancer screening, blood pressure reading, periodontal diagnosis and dental examination. The Doctor will establish a treatment plan and review any recommendations as well as answer any dental concerns that you may have.

IMPORTANT: A parent or guardian must accompany all patients under the age of 18 to this initial visit.
